Please download and save the following forms on your computer:  Application Form, Job Description, Immunisation Letter, Terms of Engagement and Equal Opportunity Policy.  Once you have completed the Application Form it can be uploaded by revisiting the Carer Application Forms/Apply Online Page and attaching your saved document.

Please complete the forms carefully, ensuring you have given a detailed work history.  When returning or uploading the forms please enclose / attach a C.V. if possible. Successful applicants will be invited to attend an interview during office hours, Monday to Friday, 8.30 a.m. to 4.30 p.m. If you have not heard from us within 10 days, please telephone.

The Health & Social Care Act 2008 requires all persons working with vulnerable people to undertake a DBS check.

Please ensure you bring the following to your interview:

1. Two passport size photographs
2. A Curriculum Vitae (C.V.) with full work history (if not sent with application forms)
3. One of the following (with a photocopy): P45, P60, a payslip or National Insurance Card
4. Your passport and a photocopy of the page showing your personal details and any relevant stamps
5. Birth Certificate or Marriage Certificate (if you have one)
6. Your Driving Licence (if you have one)
7. Two of the following showing your current address: recent bank, mortgage, insurance or credit card statement, recent gas, electric, water, phone or Council Tax bill
8. Any relevant certificates (with photocopies) that you may have (e.g. BTEC, NVQ, first aid, moving & handling, food hygiene, health & safety, fire safety, infection control etc.)
9. If you have already been immunised for Hepatitis B, please bring your confirmation letter/certificate with you.
10. If you have lived overseas, a Police Check or Certificate of Good Conduct from that country.
